Nuclear sclerotic cataracts are the most common type of age-related cataract. They form in the nucleus, or center, of the lens and develop gradually over many years. Because they progress slowly, many people are unaware they have one until a routine eye exam reveals it.
As a nuclear cataract develops, the center of the lens hardens and shifts in color from clear to yellow, and eventually to brown in more advanced cases. Your cataract surgeon can detect this yellowing during a slit-lamp examination, which uses a magnified beam of light to examine the structures of the eye.
Nuclear cataracts tend to impair distance vision more than near vision in the early stages. As the lens hardens, it can temporarily increase the eye's focusing power, sometimes improving near vision for a period. This effect, sometimes called 'second sight,' is temporary and reverses as the cataract continues to mature. You may also notice that colors appear more yellow or washed out, and that your vision is less sharp in low light.
Because nuclear cataracts progress slowly, many people manage their symptoms with updated glasses prescriptions for a period of time. Surgery becomes the right next step when your vision loss begins to affect driving, reading, or other activities that are important to you. Your cataract surgeon will monitor progression at each visit and guide you on timing.
Cortical cataracts develop in the cortex, the outer layer of the lens that surrounds the nucleus. They have a distinctive appearance and tend to cause different visual symptoms than nuclear cataracts. Certain medical conditions and environmental factors can increase the likelihood of developing this type.
Under examination, cortical cataracts appear as wedge-shaped or spoke-like white streaks that extend from the outer edge of the lens toward its center, somewhat like the spokes of a wheel. Your cataract surgeon can identify these patterns during a dilated eye exam before they begin to affect your central vision. As the spokes grow inward toward the visual center of the lens, symptoms gradually worsen.
The most noticeable symptom is glare, particularly from headlights while driving at night. Bright light scatters when it hits the spoke-like opacities, making nighttime driving uncomfortable or hazardous. UV-B light exposure and diabetes are associated with a higher risk of cortical cataracts. Color perception may fade gradually, though this change is often subtle enough that you may not recognize it until your vision is restored after surgery.
Cortical cataracts progress at varying rates. Some remain at the edge of the lens for years with little impact, while others advance toward the center relatively quickly. Anti-glare coatings on glasses may help in the short term, but surgery is the definitive treatment when glare or reduced clarity begins to limit your daily activities.
Posterior subcapsular cataracts, often called PSC, form on the back surface of the lens directly in the path of light entering the eye. Even a small clouding in this location can significantly affect vision, making this type particularly disruptive despite its size. PSC cataracts can occur in people younger than those typically affected by other cataract types.
PSC is strongly associated with long-term corticosteroid use, which includes oral, inhaled, and topical forms. Other risk factors include diabetes, prior radiation exposure to the eye area, and previous eye surgery or injury. If you use corticosteroids regularly for conditions such as asthma, arthritis, or inflammatory diseases, your cataract surgeon will monitor you closely for early signs of PSC.
Unlike nuclear cataracts, PSC tends to impair near vision and reading before it affects distance vision. Bright light causes significant glare because light converges directly through the opacity at the back of the lens. Many people with PSC find that their vision is notably worse in bright sunlight or under strong indoor lighting, which is the opposite of what typically occurs with nuclear cataracts.
PSC cataracts can progress from mild to significantly disabling within months rather than years. Because of this faster progression and the disproportionate impact on reading and bright-light vision, surgery is often recommended sooner than for other cataract types. When the surrounding capsule is intact, surgical outcomes for PSC are generally excellent.
While age-related cataracts account for the majority of cases, cataracts can also result from injury, other health conditions, and factors present from birth. Recognizing these types matters for proper diagnosis and treatment planning.
Congenital cataracts are present at birth or develop shortly after. They can result from genetic factors, chromosomal conditions, metabolic disorders, or infections during pregnancy. Early detection and prompt treatment are critical in children because an untreated cataract can interfere with visual development and lead to amblyopia, commonly called lazy eye. Pediatric cataracts require specialized surgical care and a careful rehabilitation plan following surgery.
A cataract can develop after blunt or penetrating injury to the eye, chemical burns, electric shock, or radiation exposure. Blunt trauma sometimes creates a characteristic petal or rosette-shaped cloudiness in the lens. A traumatic cataract may appear shortly after injury or develop gradually over months or years, depending on the nature and severity of the injury.
Secondary cataracts develop as a result of other eye conditions or medical treatments rather than aging alone. Chronic uveitis (ongoing inflammation inside the eye), long-term steroid therapy, and radiation treatment can all cause the lens to cloud. Your cataract surgeon will address the underlying cause alongside the cataract when planning care. It is worth noting that the term 'secondary cataract' is also used informally to describe posterior capsule opacification, a separate condition that can occur after cataract surgery and is treated with a quick laser procedure.

Yes, and it is actually quite common. Most cataracts involve a combination of types, with one pattern tending to drive the primary symptoms. During your exam, your cataract surgeon will identify which type or combination is most affecting your vision. Because surgery removes the entire natural lens at once, the presence of multiple types does not change the treatment approach.
It can. PSC cataracts often call for earlier surgery because they progress more quickly and have a greater impact on reading and functioning in bright light. Nuclear cataracts usually allow more time before surgery becomes urgent. Ultimately, the decision is based on how much your daily life is affected, not on cataract type alone. If you are unsure whether your symptoms warrant surgery, a cataract evaluation will give you a clear picture.
Yes. During a dilated slit-lamp exam, your cataract surgeon will identify the location, pattern, and density of your cataract and explain what it means for your vision and your timeline. Knowing your specific type helps you understand the symptoms you are experiencing now and what changes to expect as it progresses.
The core procedure is the same regardless of type: phacoemulsification, which uses ultrasound energy to break up and remove the clouded lens, followed by placement of an IOL. However, technical adjustments are sometimes needed. A very dense nuclear cataract may require more ultrasound energy to remove. A traumatic cataract may involve additional challenges if the lens capsule or supporting structures were damaged by the injury. Your cataract surgeon will tailor the approach to your specific anatomy and situation.
There is no proven way to stop cataracts from forming entirely, but certain habits may help slow progression. Wearing UV-blocking sunglasses outdoors, avoiding smoking, keeping blood sugar well controlled if you have diabetes, and minimizing unnecessary steroid use are all reasonable steps. Regular eye exams after age 40 allow your doctor to detect changes early, giving you more time to plan ahead before vision loss becomes significant.
After cataract surgery, some patients develop a condition called posterior capsule opacification (PCO), where cells grow on the thin membrane left behind after surgery. PCO causes blurry vision that can feel similar to a cataract returning, which is why it is sometimes called a secondary cataract. However, PCO is not a new cataract. It is treated with a brief, painless laser procedure performed in the office that clears the cloudiness in just a few minutes.
