Laser and Surgical Treatment Options

When eye drops alone are not enough, or when a patient has difficulty using drops consistently, laser treatments and surgical procedures offer additional ways to lower eye pressure. These options range from brief in-office procedures to more involved surgeries, and the right choice depends on your type of glaucoma, the level of pressure control needed, and your overall eye health.

Selective laser trabeculoplasty, known as SLT, is now considered a first-line treatment option for open-angle glaucoma (the most common form of the disease). The procedure is performed in the office using numbing drops, and most patients experience little to no discomfort. A low-energy laser is applied to the drainage tissue inside the eye to improve how freely fluid can flow out. The pressure-lowering effect builds gradually over a few weeks and can last several years. SLT can be repeated if needed and can be used alongside eye drops for added benefit.

Minimally invasive glaucoma surgery, or MIGS, refers to a group of procedures that use small tools or tiny openings to create improved drainage pathways inside the eye. These procedures heal faster and carry fewer risks than traditional glaucoma surgery, making them a strong option for mild to moderate cases. MIGS is most commonly performed at the same time as cataract surgery, allowing both problems to be addressed in a single session. For many patients, MIGS can reduce or even eliminate the need for daily eye drops after recovery.

Whether MIGS is right for you is a decision made in consultation with your surgeon, based on the type and stage of your glaucoma and your overall eye health.

When drops, laser, and minimally invasive procedures are not enough to control pressure, traditional surgery may be recommended. Trabeculectomy creates a new channel for fluid to drain out of the eye. Tube shunt surgery places a small tube that redirects fluid to a space beneath the outer surface of the eye. Both procedures can achieve greater pressure reduction than other treatments, but they involve a longer recovery and a higher chance of complications. For this reason, they are typically reserved for cases where other methods have not provided adequate control. After either procedure, regular follow-up visits remain important to ensure the drainage pathway stays open and pressure stays within a safe range.

A newer category of glaucoma treatment involves small implants placed inside the eye during a brief office procedure. These implants slowly release a pressure-lowering medication over an extended period, removing the need for daily drops for as long as the implant remains active. This approach is particularly helpful for patients who have difficulty keeping up with a strict drop schedule. Research into additional implant designs is ongoing, and this area of treatment continues to expand.

Monitoring Your Glaucoma Over Time

Glaucoma is a lifelong condition that requires consistent follow-up care. Regular monitoring allows your eye doctor to confirm that treatment is working, catch any changes early, and adjust the plan before significant vision loss occurs. Keeping your scheduled appointments is just as important as taking your prescribed medication.

Patients being treated for glaucoma typically need follow-up visits every three to six months, depending on how stable the condition is. At each appointment, your eye doctor will measure your eye pressure, examine the optic nerve, and may perform additional imaging or visual field testing. These routine checks are what allow subtle changes to be detected before they become noticeable to you.

If side effects are making it hard to stay consistent with your drops, a gentler formulation or a different drug may be tried. The goal is always to find the plan that keeps your pressure under control with the fewest possible side effects. Sharing your honest experience at each visit helps your care team make the most informed decisions for you.

Living Well With Glaucoma

A glaucoma diagnosis is understandably concerning, but most people who receive consistent care maintain good quality of life and protect their vision for many years. Understanding your condition and staying engaged with your treatment plan are the most important things you can do.

Most people with chronic open-angle glaucoma who receive steady treatment do not go blind. Early detection, regular follow-up, and consistent use of prescribed therapy give you the best chance of keeping your vision stable over the long term. While glaucoma does not have a permanent cure, it is a manageable condition when approached with ongoing care and attention.

Healthy lifestyle habits can complement your medical treatment, though they do not replace it. Moderate aerobic exercise may help lower eye pressure slightly. A diet rich in leafy greens and omega-3 fatty acids supports overall nerve health. Staying well hydrated is beneficial, but drinking a very large amount of fluid in a short period of time can cause a brief spike in eye pressure. Good sleep and stress management also play a supporting role in overall health.

Managing a lifelong condition can feel overwhelming at times. Asking questions at every visit, learning more about your specific type of glaucoma, and connecting with others through patient support groups can all help reduce anxiety. For patients who have already experienced some loss of peripheral vision, low-vision tools such as magnifiers, improved lighting, and occupational therapy can make daily tasks easier. Your eye doctor can point you toward resources and community organizations that offer guidance and peer connection.

Frequently Asked Questions

Below are answers to common questions about glaucoma treatment, monitoring, and daily life. These responses are meant to add practical guidance beyond what is covered in the sections above.

Current treatments cannot reverse damage that has already occurred to the optic nerve. The focus of all treatment is to slow or stop further loss. Starting treatment before significant damage has occurred gives the best chance of maintaining useful vision long term, which is why regular eye exams are so valuable even when you have no noticeable symptoms.

That depends on how your eye responds to the procedure. Some patients achieve enough pressure reduction from SLT alone to discontinue drops, while others continue to need one or more medications. Your eye doctor will monitor your pressure closely in the weeks following the laser and use those results to guide the next step. It is important not to stop drops on your own before getting that follow-up confirmation.

Cataract surgery is generally considered safe for glaucoma patients and can even produce a modest reduction in eye pressure on its own. When appropriate, a MIGS procedure can be combined with cataract removal in the same session to provide additional pressure control. Whether a combined approach makes sense for you depends on your glaucoma type and stage, the severity of the cataract, and your overall eye health. Your surgical team will walk you through the benefits and risks before any decision is made.

Glaucoma rarely causes noticeable symptoms until late in the disease, which is why monitoring tests matter so much. A rising pressure reading, a new change on a visual field test, or evidence of nerve fiber thinning on an OCT scan may indicate that the current plan needs adjustment. If you are also experiencing side effects that are making it hard to use your drops consistently, that is equally important to bring up at your next visit, since missed doses can allow pressure to climb without you feeling any different.

Moderate aerobic exercise is generally beneficial and may produce a small reduction in eye pressure. However, certain activities can temporarily raise pressure, including heavy weight lifting with breath-holding and inverted yoga poses where the head is positioned below the heart. These activities do not necessarily need to be eliminated, but you should discuss your specific fitness routine with your eye doctor so they can give you personalized guidance based on the severity of your glaucoma.

MIGS is generally best suited for patients with mild to moderate glaucoma, particularly those who are already planning to have cataract surgery. It is not typically recommended for advanced cases where more aggressive pressure reduction is needed. The decision involves a detailed review of your eye anatomy, pressure history, medication use, and overall goals for treatment.
