Ptosis: Understanding and Treating a Droopy Eyelid
What Causes a Drooping Eyelid
Ptosis has several distinct causes, and identifying the correct one shapes the entire treatment plan. Your Oculoplastic Surgeon will examine your eyelid carefully to determine what is driving the droop.
The most common type of ptosis in adults is called aponeurotic ptosis. The levator muscle is responsible for lifting your upper eyelid, and it connects to the lid through a thin tendon called the aponeurosis. Over time, that tendon can stretch or partially detach from its anchor point. The muscle itself still functions, but the loosened connection causes the lid to hang lower.
Long-term contact lens wear, prior eye surgery, and the natural aging of tissues are all recognized contributors to this type of ptosis.
Congenital ptosis occurs when a baby's levator muscle does not develop fully. Instead of normal muscle fiber, the tissue is fibrous and does not stretch or contract the way healthy muscle does. Parents often notice one lid sitting lower in early photographs.
Children born with a significant droop need prompt evaluation. If the lid covers the pupil during the years when the visual system is still forming, the brain can permanently reduce its reliance on that eye, leading to lifelong vision problems.
Several less common but important causes of ptosis exist, and your evaluation will help identify which applies to your situation.
- Neurogenic causes: third nerve palsy, Horner syndrome, and Marcus Gunn jaw-winking syndrome
- Myogenic causes: myasthenia gravis and other muscle diseases where lid position worsens as the day progresses
- Mechanical causes: a stye, cyst, or growth that physically weighs the lid down
- Traumatic causes: injury or post-surgical swelling that damages the levator muscle or its attachment
How Ptosis Is Evaluated
A thorough evaluation is essential before any treatment is recommended. The measurements taken during your visit directly guide both the surgical plan and any insurance documentation that may be needed.
Your Oculoplastic Surgeon measures the distance from the center of your pupil to the edge of your upper eyelid, a value called the margin reflex distance. This tells them how severe the droop is. They also measure the total travel of your lid from looking all the way down to looking all the way up, which reflects the strength of the levator muscle and helps determine the best surgical approach.
If the droop appeared suddenly or comes with other symptoms, additional testing may be needed. Your doctor may order blood work to screen for myasthenia gravis, imaging of the brain and eye socket (orbit), or a referral to a neurologist. These steps confirm whether the ptosis has a nerve or brain-related cause that requires its own separate treatment.
You may be asked to complete a visual field test, where you look at a screen and respond to flashes of light in different areas of your vision. The test is done twice: once with your lid in its natural position, and once with it gently taped up. This comparison shows how much of your upper visual field the droop is blocking and is frequently used by insurance carriers to determine whether ptosis repair qualifies as medically necessary.
Treatment Options for a Droopy Eyelid
The right treatment for ptosis depends on the underlying cause, the severity of the droop, and your age. Your Oculoplastic Surgeon will review all options with you and help you choose the approach that fits your situation best.
Oxymetazoline 0.1% is an FDA-approved prescription drop for adults with acquired ptosis. It works by stimulating a small additional muscle in the eyelid called Muller's muscle, producing a modest lift. The effect begins within about 15 minutes and lasts several hours. Eye drops are best suited for mild droops in adults and are not appropriate for children or more significant cases.
For most adults with age-related ptosis, the standard surgical approach involves tightening the stretched levator tendon through a small incision placed in the natural eyelid crease. You remain awake with local anesthesia, and your surgeon may ask you to open and close your eyes during the procedure to fine-tune the lid height. The incision heals within the crease and becomes difficult to notice once fully healed.
Ptosis repair is performed as an outpatient procedure, meaning you go home the same day. Bruising and swelling are expected for one to two weeks, and final lid position settles over about two to three months.
When the levator muscle is severely weak, as in some congenital cases or certain muscle diseases, directly tightening it may not be possible or sufficient. In these situations, the surgeon creates a sling that connects the eyelid to the forehead muscle, so the lid rises when you raise your brow. The sling can be made from silicone, a portion of your own tendon, or other suitable materials.
Many patients with a drooping lid also have excess upper eyelid skin, a condition called dermatochalasis. In these cases, the surgeon can tighten the levator tendon and remove the loose skin through the same crease incision in a single procedure. Combining both steps at once often produces a more balanced and natural result than separating them into two surgeries.
An additional internal approach called Muller's muscle-conjunctival resection involves no skin incision and is a good option for mild droops when testing shows the lid responds well to drops.
What to Expect During Recovery
Recovery after ptosis repair is manageable for most patients, and knowing what to expect at each stage helps the process feel more predictable. Following your surgeon's post-operative instructions closely gives you the best chance of a smooth, comfortable recovery.
Swelling, bruising, and a tight feeling around the eyelid are normal in the first several days. Use cool compresses for the first 48 hours, keep your head elevated when resting, and avoid bending over or lifting heavy objects. Apply antibiotic ointment to the incision as directed, and take acetaminophen for mild discomfort if needed.
Most adults are comfortable returning to desk work within 7 to 10 days. Bruising fades and the crease starts to look more natural. Avoid eye makeup until your surgeon gives the go-ahead, and hold off on heavy exercise, swimming, and contact lens wear for approximately two weeks. Some asymmetry between the two lids during this phase is normal, as each side heals at its own pace.
Final lid height is typically visible by three months after surgery. A minor adjustment is occasionally needed if one lid settles higher or lower than planned. Results are long-lasting for most patients, though the eyelid tissues continue to age, and a future revision may be considered years or decades down the road.
Ptosis in Children
Childhood ptosis carries unique risks because the visual system is still actively developing. Prompt evaluation and careful timing of treatment are essential to protecting a child's long-term vision.
A child's visual system develops through approximately age 8. If a significant droop covers the pupil during this window, the brain can stop relying on that eye, resulting in amblyopia (lazy eye) that may be permanent without treatment. The drooping lid can also press against the cornea, the clear front surface of the eye, causing astigmatism that blurs vision even after the lid is corrected.
For a severe droop that is blocking the pupil, surgery is often recommended within the first year of life to protect developing vision. For a mild to moderate droop that is not obstructing sight, many surgeons wait until age 3 to 5, when measurements are more reliable and the child can better participate in the exam. A pediatric eye specialist guides the timing decision based on the individual child's situation.
Parents often have concerns about anesthesia, scarring, and how their child will handle the experience. Pediatric anesthesia for eyelid surgery is a routine and well-established practice, and the incision heals within the natural lid crease. It is equally important to keep up with post-surgical vision check-ups, because patching or prescription drops for amblyopia may still be needed even after a successful repair.

Insurance typically covers ptosis repair when a visual field test documents that the droop is blocking your upper field of vision. Purely cosmetic lid procedures are not covered under most health insurance plans, so clarifying the goal with your surgeon before surgery helps set the right expectations.
A droop that worsens with fatigue is a recognized warning sign of myasthenia gravis, an autoimmune condition that weakens the connection between nerves and muscles over repeated use. If your surgeon suspects this, a blood test and an ice test (where placing ice on the lid briefly may improve the droop) can help confirm it. The treatment for myasthenia gravis is medical rather than surgical, so identifying this cause before any procedure is important.

A droop that develops over minutes to hours, particularly when accompanied by a pupil that looks different in size, double vision, a severe sudden headache, or weakness or numbness anywhere in the body, should be treated as an emergency. These signs may indicate a stroke, brain aneurysm, or a third nerve palsy, all of which require immediate imaging and evaluation. Do not wait for a scheduled office visit in this situation.
Most patients enjoy long-lasting improvement, often a decade or more, before any loosening becomes noticeable. Factors that can shorten the duration include continued contact lens wear, frequent eye rubbing, and having the surgery at a younger age. If the droop returns, a revision procedure can retighten the tendon, and many patients who need a touch-up find it is a straightforward outpatient visit similar to the original surgery.
Ptosis involves a weakened or stretched muscle or tendon and causes the lid margin to sit too low, sometimes covering the pupil. I've been seeing Dr. Reisner for over 20 years for all of my eye care needs. He did my Lasik surgery 20 years ago, treated my extremely dry eye issues, took care of my eyeglass prescriptions, did both of my cataract surgeries, and today surgically removed a calcification from my lower eyelid.
