Understanding Pediatric Eye Care

A Pediatric Ophthalmologist is a medical doctor who completed advanced fellowship training specifically in diagnosing and treating eye conditions in children. This specialty covers both medical and surgical eye care for children, as well as strabismus (eye misalignment) evaluation and treatment across all age groups.

A child's visual system is still forming throughout childhood, which means eye problems during these years can have lasting effects on how the brain processes sight. Pediatric ophthalmology exists to detect and treat these problems early, when intervention is most effective and the visual system is still adaptable.

How It Differs from General Eye Care

General ophthalmologists and optometrists provide excellent care for many eye conditions. A Pediatric Ophthalmologist brings additional expertise in how a child's visual system grows and changes. That includes specialized exam techniques for infants and toddlers, smaller surgical instruments designed for young patients, and deep familiarity with conditions that appear only during childhood.

Children are not simply small adults when it comes to eye care. Their refractive errors shift as they grow, their brains are still forming visual connections, and many childhood eye conditions have narrow treatment windows. Seeing a specialist trained in these differences ensures your child receives the most accurate and timely care.

Warning Signs to Watch For

Many childhood eye conditions do not cause pain and may not be obvious to parents without specialized testing. If your child's pediatrician recommends a referral, it is worth following through even when your child appears to see well.

Eyes that cross or drift, especially when it is constant or continues after four months of age
A white or yellowish pupil (leukocoria)
Failure to track objects or make eye contact by three months
Persistent tearing or eye discharge
Squinting, head tilting, or closing one eye
Sitting very close to screens or holding books very near the face

Age-Appropriate Vision Testing

A visit to our Pediatric Ophthalmologist is designed with your child's comfort and cooperation in mind. We adapt every part of the exam to your child's age and developmental stage, using techniques and tools specifically designed for young patients.

What to Expect

Beyond vision testing, a full pediatric evaluation includes a dilated examination and any measurements your child's situation calls for.

The Dilated Eye Exam

Dilating drops widen the pupil so we can examine the retina, optic nerve, and internal structures of the eye. The drops may cause blurred near vision and increased light sensitivity that can last several hours, and occasionally longer depending on the drops used. We recommend bringing sunglasses for your child and planning lighter close-up activities for the rest of the day.

The dilated exam helps us detect cataracts, retinal conditions, optic nerve abnormalities, and other concerns that cannot be seen without dilation. The drops cause a brief stinging sensation when applied, but the exam itself is completely painless and provides information that no other method can replace.

Measurements and Special Tests

We measure eye alignment using cover tests and prism assessments, and we check eye pressure when glaucoma is a concern. Optical coherence tomography (OCT), a non-invasive imaging scan, captures detailed images of the retina. Electroretinography (ERG) tests how the retina responds to light and is used when inherited retinal conditions are suspected. We order only the tests relevant to your child's situation and explain every step before we begin.

When Should Your Child See a Pediatric Ophthalmologist?

Some children are referred by their pediatrician after a failed vision screening, while others come to us because a parent noticed something concerning. Either way, early evaluation gives your child the best chance at healthy visual development.

Your child's pediatrician performs basic vision screening at well-child visits, which is an important first step. Professional organizations including the American Academy of Ophthalmology, the American Academy of Pediatrics, and AAPOS support routine vision screening throughout childhood. If any concern is identified during screening, a referral to a Pediatric Ophthalmologist allows for a thorough and accurate evaluation that goes beyond what a screening tool can detect.

A comprehensive pediatric eye exam is also recommended when there is a family history of childhood eye disease, a history of premature birth, or a developmental condition associated with higher rates of eye problems.

Children develop a wide range of eye conditions that require specialized knowledge and care. Our Pediatric Ophthalmologist is trained to manage everything from common refractive errors to complex surgical conditions present from birth.

Amblyopia (Lazy Eye)

Amblyopia is the most common cause of reduced vision in one eye in children and young adults. It occurs when the brain begins to favor one eye, causing the weaker eye's vision to fall behind. Treatment with glasses, patching, or atropine eye drops trains the brain to use the weaker eye, and early intervention produces the best outcomes.

Strabismus (Eye Misalignment)

Strabismus causes one eye to point in a different direction from the other. We evaluate the type, frequency, and degree of misalignment and determine whether glasses, patching, botulinum toxin injections that relax an overactive eye muscle, or strabismus surgery is the right approach. Left untreated, strabismus can lead to amblyopia and affect a child's depth perception and confidence.

Refractive Errors in Children

Nearsightedness, farsightedness, and astigmatism are among the most common eye conditions in children. Young children often cannot report blurry vision because they have no point of comparison. We detect refractive errors through retinoscopy in infants and toddlers and standard refraction testing in older children. A significant difference between the two eyes, called anisometropia, can lead to amblyopia if left uncorrected.

Blocked Tear Ducts

Infants with a blocked duct experience constant tearing and sticky eye discharge, even without infection. Most cases resolve on their own by the first birthday with regular massage techniques we teach parents at home. When the blockage persists, a brief probing procedure opens the drainage pathway and typically resolves the problem quickly.

Most children we see have common, treatable conditions. We are also equipped to manage rarer and more serious ones present from birth or developing during childhood, including congenital cataracts, pediatric glaucoma, retinopathy of prematurity, retinoblastoma (a rare eye cancer), and inherited retinal and genetic eye diseases. Children Who Benefit from Early Specialist Care

Do I need a referral to schedule an appointment?

Some insurance plans require a referral from your pediatrician before seeing a specialist, so it is worth checking with your insurance provider in advance. For urgent concerns such as a white pupil, sudden vision loss, eye injury, or new eye turning, we encourage you to contact us right away rather than waiting for a referral to be processed. Prompt evaluation in these situations is important.

How is a Pediatric Ophthalmologist different from an optometrist who sees children?

A Pediatric Ophthalmologist is a medical doctor with surgical training who can diagnose, medically manage, and operate on eye conditions in children. An optometrist provides valuable vision testing, glasses prescriptions, and management of non-surgical conditions. Some children benefit from seeing both types of providers at different points in their care, and we are happy to coordinate with your child's optometrist when appropriate.

Can my child just see our family eye doctor?

General eye care providers handle many routine needs for children very well. A Pediatric Ophthalmologist becomes especially important when your child's condition requires a level of specialized evaluation, diagnosis, or treatment, including surgery, that goes beyond general practice. Your pediatrician's referral is usually the clearest signal that a specialist visit is the right next step.

When does my child transition to an adult eye doctor?

Many Pediatric Ophthalmologists continue to see patients through their late teens or into their early twenties, particularly when an ongoing condition like strabismus or amblyopia is being actively managed. The timing of transition depends on the diagnosis, how stable the condition is, and whether an adult specialist with relevant experience is available. We help families plan this transition thoughtfully when the time comes.

How often will my child need follow-up visits?

Visit frequency depends entirely on the diagnosis and stage of treatment. Children actively being treated for amblyopia or strabismus often return every four to eight weeks so we can monitor progress and adjust treatment. Children with stable conditions may need only one or two visits per year. Once a child's exams are normal and no active treatment is needed, routine screening through their pediatrician may be sufficient. We give you a clear follow-up plan at each visit.

Are glasses always required after a pediatric eye exam?

Not always. Some children have normal refractive errors that do not require correction, while others have prescriptions significant enough to affect vision development or learning. When glasses are recommended, it is because they serve a medical purpose, whether that is preventing amblyopia, reducing eye strain, or helping the eyes work together more comfortably. We explain exactly why glasses are being prescribed so you understand what they are doing for your child's vision.

He did my Lasik surgery 20 years ago, treated my extremely dry eye issues, took care of my eyeglass prescriptions, did both of my cataract surgeries, and today surgically removed a calcification from my lower eyelid. He is an excellent doctor in every way (kind, patient, great listener, etc.). Staff are always kind and professional I absolutely recommend Dr. Reisner and The Eye Center. Update: Another visit (Fairfax office) and saw Dr. Tailor for a stye problem She is friendly, patient and answered all of my questions. I did not feel rushed at all. Staff wonderful.
