What to Expect During a Gonioscopy Exam
Gonioscopy is a brief, well-tolerated office procedure. Knowing what will happen from start to finish can help you feel at ease on the day of your exam.
We will place numbing drops in your eye before we begin. These drops may sting for a second or two, but they quickly make your eye comfortable for the rest of the procedure. You do not need to fast or avoid your usual medications beforehand.
Gonioscopy is generally performed before any pupil dilation, because dilating drops can actually narrow the angle or trigger sudden blockage in certain eyes. Please let us know about any medications you take and any known allergies to eye drops so we can prepare accordingly.
A goniolens is a specially designed mirrored contact lens that allows us to look around the curved structures inside your eye. We apply a small amount of clear gel to the lens surface to help it sit comfortably on your numbed eye. You will rest your chin and forehead against our slit lamp, the same magnifying instrument used in routine eye exams.
The mirrors inside the goniolens reflect light from different angles so we can view the entire drainage angle. You may notice brief flashes of light as we gently rotate the lens, but the exam is not painful.
Most patients feel only a mild sense of pressure when the goniolens makes contact with the eye. Because the numbing drops are working, you should not feel any sharp pain. You might see colorful reflections or notice that the lens temporarily fills part of your field of view, but these sensations disappear as soon as the lens is removed.

The gonioscopy exam itself usually takes about five to ten minutes. We examine all quadrants of the drainage angle in each eye, rotating the lens carefully to get a complete picture. If we find an area that needs closer inspection, the exam may run a few minutes longer.
When gonioscopy is combined with numbing drops and any additional components of your comprehensive eye exam, you should plan to spend about twenty to thirty minutes at our office for the full visit.
The most common approach is indirect gonioscopy, performed at the slit lamp while you sit upright. This method is comfortable, fast, and allows us to perform special maneuvers to check whether a narrow angle can be reversed. Direct gonioscopy, which requires you to lie down, is typically reserved for specialized situations such as examining infants or performing angle assessments during surgery.
We use indirect gonioscopy for nearly all routine evaluations because it provides excellent visualization of the angle and is well tolerated by the vast majority of patients.
Understanding Your Gonioscopy Results
The findings from your gonioscopy exam directly guide how we manage your eye pressure and glaucoma risk. Here is what the different results mean and how they shape your care.
An open angle means we can clearly see all the normal drainage structures and the iris is not blocking the pathway. This is an encouraging finding and tells us the angle is not anatomically closed. However, glaucoma can still develop in open-angle eyes if the drainage tissue is not functioning efficiently, so we continue to monitor your pressure and optic nerve on a regular schedule.
People with open angles typically develop glaucoma more gradually and can often be managed with pressure-lowering eye drops, laser treatment, or a combination of both.
A narrow angle means the space between your iris and cornea is smaller than normal, leaving the drainage structures partially hidden. We grade how much of the trabecular meshwork is visible and assess how close the iris sits to the drainage tissue. A narrow angle increases the risk of sudden or gradual blockage, particularly in dim light or when the pupil is dilated.
We also evaluate whether the narrowing is appositional, meaning the iris is close but can be pushed away, or synechial, meaning scar tissue has created permanent attachments. This distinction is important because it influences whether preventive treatment is needed and which approach is most appropriate for your situation.

A closed angle means the iris is pressing against the drainage area and blocking fluid from leaving the eye. This can occur suddenly as an acute emergency or develop slowly over months and years. Acute angle closure causes severe eye pain, nausea, and rapid vision loss and requires immediate emergency care.
Several mechanisms can cause angle closure, including pupillary block (the most common), plateau iris configuration, or a lens that has shifted forward. Treatment depends on the underlying cause and typically begins with medications to lower pressure quickly, followed by a laser or surgical procedure tailored to your specific anatomy. In many cases, a laser peripheral iridotomy, which creates a small opening in the iris to allow fluid to flow around it, can prevent future episodes.
While examining your drainage angle, we also look for abnormal blood vessels that can grow across the angle in conditions like diabetes or retinal vein blockages, as well as pigment deposits, scar tissue, and signs of prior inflammation. These findings help explain patterns of elevated eye pressure that other tests alone cannot fully account for.
We may also identify signs of pseudoexfoliation syndrome, pigment dispersion syndrome, or angle recession from a prior eye injury. Recognizing these conditions early allows us to choose the most effective treatment and anticipate how your eye pressure may behave over time.
After Your Gonioscopy
Recovery after gonioscopy is straightforward for most patients. A few simple precautions will keep your eyes comfortable and safe in the hours that follow.
Your vision may be slightly blurry right after the exam from the gel used with the goniolens. This clears within a few minutes as you blink and your tears wash the gel away. If your pupils were dilated for another part of your exam, your near vision will remain blurry and you may be sensitive to bright light for several hours.
Do not rub or press on your eye until the numbing effect has fully worn off, which typically takes about twenty to thirty minutes. While your eye is numb, you could accidentally scratch the surface without feeling it. If you wear contact lenses, wait until your eye feels fully normal before reinserting them, or follow the specific timeline we give you at your visit.
Most patients have no side effects beyond brief blurring and mild light sensitivity. The numbing drops wear off quickly, and any gritty or scratchy feeling from the contact lens usually settles within an hour or two.

Serious complications are rare. We follow strict disinfection protocols for all lenses used in the exam. In uncommon cases, patients may experience a small corneal scratch, a mild allergic reaction to the numbing drops or gel, or a brief feeling of lightheadedness during the procedure. These issues are typically minor and resolve quickly.
If you have narrow angles or received dilating drops, watch carefully for warning signs of acute angle closure in the hours after your visit. Severe headache, nausea or vomiting, rapidly worsening vision, intense halos around lights, or a red and painful eye are signs of a possible emergency that requires immediate care.
Please contact us right away or seek urgent eye care if you experience sudden severe eye pain, worsening redness beyond a few hours, or unusual discharge. New flashes of light, floaters, or a shadow across your vision are unrelated to gonioscopy but also require immediate evaluation for potential retinal problems. If symptoms are severe and occur after hours, go to an emergency department with eye care capability rather than waiting.
We may schedule a follow-up gonioscopy if your angle was borderline between open and narrow, or if changes in your eye pressure or optic nerve suggest the angle should be reassessed. Patients who have had laser or surgical treatment on the drainage angle are monitored periodically to confirm the procedure continues to be effective.
If your angles are healthy and open, we will continue routine eye care with regular pressure monitoring. Narrow angles may lead us to recommend a laser peripheral iridotomy, a short outpatient procedure that creates a tiny opening in the iris to keep fluid flowing safely and reduce your risk of angle closure. Closed angles or other significant findings may require additional testing, medications, or surgery, and we will walk you through every option so you can make an informed decision about your care.

For most patients, the exam is not painful at all. The numbing drops do sting for a second when first applied, but once they take effect you will only feel a gentle pressure when the lens contacts your eye. If you find yourself tensing up or feel discomfort during the exam, let us know so we can pause and make sure you are as comfortable as possible before continuing.
If no dilating drops were used and your vision feels clear and steady, driving is generally fine. However, if we dilated your pupils as part of a comprehensive exam, you should plan for a ride home because your near vision will be blurred and glare from sunlight or headlights can make driving unsafe. It is always a good idea to ask us at the end of your visit whether dilation was used so you can plan accordingly.
A standard eye pressure measurement tells us the level of pressure inside your eye but gives no information about why it may be elevated or what the drainage angle looks like. Gonioscopy is the only test that allows us to directly view the angle and classify whether glaucoma is open-angle, narrow-angle, or caused by a secondary condition. Our computerized glaucoma testing provides additional detail about how the optic nerve and visual field are functioning, and gonioscopy completes the picture by revealing the structural cause of any pressure problem.
Gonioscopy is generally covered by medical insurance when it is performed to evaluate or manage elevated eye pressure, glaucoma, or related conditions. Because it is a diagnostic medical procedure rather than a routine screening, coverage typically applies when there is a clinical reason for the exam. Gonioscopy itself does not prevent glaucoma, but it enables us to identify people who are at high risk before damage occurs. The frequency depends on your individual findings and risk profile. I've been seeing Dr. Reisner for over 20 years for all of my eye care needs. He did my Lasik surgery 20 years ago, treated my extremely dry eye issues, took care of my eyeglass prescriptions, did both of my cataract surgeries, and today surgically removed a calcification from my lower eyelid. He is an excellent doctor in every way (kind, patient, great listener, etc.). Staff are always kind and professional I absolutely recommend Dr. Reisner and The Eye Center. Update: Another visit (Fairfax office) and saw Dr. Tailor for a stye problem She is friendly, patient and answered all of my questions. I did not feel rushed at all. Staff wonderful.
