The relationship between dryness and watery eyes is one of the most misunderstood aspects of eye health. Once you understand the cycle that dry eye creates, it becomes clear why treating dryness is also the key to stopping unwanted tearing.
A healthy tear film is made of three distinct layers working together. The outer oil layer prevents tears from evaporating too quickly. The middle water layer provides moisture, oxygen, and nutrients to the eye's surface. The inner mucus layer helps tears spread evenly and stick to the eye. When any of these layers breaks down, your tear film becomes unstable and evaporates too fast, setting off the reflex tearing response.
Your eye contains nerves that constantly monitor moisture levels on the surface. When these nerves detect dryness, they trigger your tear glands to produce a surge of watery tears. The problem is that these reflex tears are mostly water and salt. They lack the oils and proteins that make tears stable, so they evaporate quickly, and the dryness returns almost immediately. The meibomian glands are tiny oil-producing glands located along the edges of your eyelids. Many patients describe it as feeling like there is sand or grit in their eye even when nothing is actually there. This uncomfortable sensation is what triggers the eye to produce reflex tears.
Your tear film acts like a smooth optical surface over your eye. When it becomes unstable, light does not focus evenly, and vision can blur temporarily. You may notice that blinking clears your vision for a few moments, only for it to blur again. This fluctuation tends to be most noticeable during tasks that require sustained focus, such as reading, driving, or working at a screen.
Chronic dryness causes low-level inflammation on the eye's surface, which makes blood vessels more visible and gives the eye a red or pink appearance. Sensitivity to bright light, sunlight, and wind is also common, because the eye's surface is less protected without a stable tear film. Patients often notice their eyes tear immediately when stepping outside into a sunny or breezy environment.
Some patients notice strands or clumps of mucus collecting in the corners of their eyes or along their lashes, particularly upon waking. This indicates an imbalance in the tear film's mucus layer. A sensation of something stuck in the eye, even when nothing is present, is another signal that the eye's surface is irritated and dry. Both of these symptoms can trigger additional reflex tearing as your eye attempts to flush away the irritation.
A thorough evaluation is essential to determine whether dry eye, blocked drainage, or another condition is responsible for your tearing. Our team uses a combination of conversation, clinical testing, and careful examination to find answers.
We start by asking detailed questions about your symptoms, including when tearing occurs, what makes it better or worse, and how it affects your daily life. We will also ask about your work environment, screen habits, contact lens use, medications, and any systemic health conditions. Medications such as antihistamines, antidepressants, and blood pressure treatments can reduce tear production and contribute to dry eye-related tearing.
We use clinical tests to measure both the quantity and quality of your tears. The Tear Break-Up Time test measures how long your tear film remains stable on the eye's surface before it begins to break apart. A break-up time of less than ten seconds suggests poor tear quality. The Schirmer test uses a small paper strip to measure how much tear volume your eyes produce over a set period. Together, these tests help clarify whether your tearing stems from low tear production, unstable tears, or both.

Over-the-counter artificial tears are often the first step in managing dry eye-related tearing. Different products vary in thickness and ingredients, and some newer options include lipid components that help restore the oil layer of the tear film. Thicker gels or ointments can be useful at bedtime when the eye has extended exposure without blinking.
Applying a warm compress to closed eyelids for several minutes softens hardened oils in the meibomian glands and helps them release more effectively. Following the warm compress with gentle eyelid massage encourages oil flow along the lid margin. When inflammation is disrupting tear production, prescription drops may be recommended. Cyclosporine A and lifitegrast are two prescription medications that work by reducing the chronic inflammation underlying dry eye disease rather than simply supplementing tears. These treatments typically take several weeks to months to show their full benefit and are most effective as part of a longer-term management plan.
Punctal plugs are tiny, soft devices that are placed into the tear drainage openings to slow the outflow of tears from the eye's surface. By keeping your natural tears in contact with the eye longer, plugs reduce both dryness and the reflex tearing it triggers. Insertion is a quick, in-office procedure that is generally well tolerated. Dissolvable temporary plugs are sometimes used first to confirm that plugs provide benefit before considering a longer-lasting option.
Small but consistent changes to your daily environment can have a meaningful impact on tear film stability. Using a humidifier indoors, particularly in winter, adds moisture to dry air. Taking regular breaks from screens, blinking fully and consciously during focused tasks, and wearing wrap-around sunglasses outdoors all help reduce tear evaporation. Staying well hydrated and incorporating omega-3 fatty acids from foods like salmon, sardines, flaxseed, and walnuts may also support tear quality and reduce ocular inflammation.
