Ectropion can develop for several different reasons, and identifying the underlying cause is the first step toward choosing the right repair. Our Oculoplastic Surgeon carefully evaluates each patient before recommending any approach.
The most common form is involutional ectropion, which develops as the tendons anchoring the eyelid at the inner and outer corners gradually stretch with age. As the skin and muscle lose tone and elasticity, gravity pulls the lid downward and outward. This type is more common in older adults and tends to progress slowly over time.
Cicatricial ectropion occurs when tight scar tissue on the skin below the lashes shortens the outer layer of the lid and prevents it from resting against the eye. Several conditions and treatments can trigger this type of scarring.
Paralytic ectropion develops when the facial nerve stops working properly, as can happen with Bell palsy or after tumor surgery. Without nerve input, the orbicularis muscle (the muscle that closes the eyelid) can no longer hold the lid in position. Mechanical ectropion occurs when the weight of a cyst, growth, or significant swelling physically pushes the lid outward. In both situations, treating the root cause is a core part of the repair plan.
Every ectropion case is different. Our Oculoplastic Surgeon takes time to understand not just the appearance of your lid, but the specific reason it has turned out, before designing your treatment.
During your consultation, your Oculoplastic Surgeon gently pulls the lower lid away from the eye to measure how loose it is and watches how quickly it springs back. This snap-back test gives important information about tissue quality. Your surgeon also examines the skin for scarring, checks for any growths, tests facial nerve strength, and carefully inspects the cornea for dry spots or areas of erosion.
There is no single procedure that fits every patient. The approach is chosen based on what is actually driving the lid outward.
Before surgery, your surgeon reviews all of your current medications, particularly blood thinners, and may advise pausing certain ones with approval from your primary care physician. Photographs and precise measurements of the lids are taken. If the cornea already shows signs of dryness or irritation, you may be asked to use preservative-free lubricating drops and nighttime ointment in the weeks before your procedure to protect the eye surface.
Ectropion repair is performed as an outpatient procedure, meaning you go home the same day. The specific technique depends on the type of ectropion identified during your evaluation.
For age-related lid laxity, the lateral tarsal strip is the most frequently used approach. Your Oculoplastic Surgeon shortens the outer edge of the eyelid and reattaches it at a higher, tighter position against the bone at the outer corner of the eye socket. The procedure typically takes around 45 minutes, and the small incision is hidden within the natural crease at the outer corner of the eye.
When tight scar tissue is pulling the lid downward, simply releasing the scar may not be enough to keep the lid in place. In these cases, a thin skin graft, often taken from behind the ear, is placed into the area to fill the gap and give the lid the extra tissue length it needs. The area where the graft is taken heals on its own with a small, well-hidden scar.
When the inner corner of the lid droops and the punctum has lost contact with the eye, your surgeon may tighten the medial canthal tendon (the internal anchor at the inner corner) and reposition the punctum so tears can drain normally again. This step is often combined with a lateral tarsal strip in more severe cases. If scarring has narrowed or closed the punctum, additional repair to open the drainage opening may also be performed.
Most ectropion repairs are performed using local anesthesia with intravenous sedation. This means the area is fully numb and you remain comfortable throughout the procedure without needing general anesthesia. You will be ready to go home within one to two hours of the procedure. Because of the sedation, you will need a trusted person to drive you home and stay with you for the rest of the day.
Recovery is straightforward for most patients. Knowing what to expect at each stage helps you plan ahead and heal comfortably.
Bruising and swelling are most noticeable in the first 48 hours and gradually fade over the following week. Cool compresses applied gently to the area and keeping your head slightly elevated while sleeping both help reduce swelling. You will apply antibiotic ointment along the incision and use lubricating drops on the eye as directed. Most mild discomfort responds well to acetaminophen. Avoid rubbing the eye or bending forward at the waist during the first week.
Sutures either dissolve on their own or are removed at your follow-up appointment around five to ten days after surgery, depending on the type used. Most patients feel comfortable returning to desk work within a week and resume full activity by two weeks. The outer corner of the lid may appear slightly elevated at first and the tissue may feel tight as healing progresses. Both of these sensations soften as swelling continues to resolve.
By three months after surgery, the lid typically sits in its correct position against the eye, tears drain through the punctum rather than spilling down the cheek, and the raw, irritated feeling is gone. Any skin graft blends closer to the surrounding skin tone by six months. A well-performed repair holds for many years, though the natural aging process continues and some patients may benefit from a minor adjustment a decade or more later.
Certain cases of ectropion require additional planning and coordination. Our Oculoplastic Surgeon has experience managing these more complex presentations with a thorough, individualized approach.
When the facial nerve does not fully recover after Bell palsy or another nerve injury, the lower lid can remain chronically weak and turned outward. A lateral tarsal strip combined with a small platinum or gold weight placed in the upper eyelid can restore the ability to close the eye fully and prevent serious corneal damage. Surgery is generally delayed until nerve recovery has reached a stable plateau, which often takes six to twelve months after the initial injury. Your Oculoplastic Surgeon may coordinate with your neurologist during this monitoring period.
Lower lid ectropion that develops after excision of a skin cancer near the eyelid is a common reason patients seek revision surgery. The tight scar left behind after cancer removal prevents the lid from sitting correctly, and a skin graft is almost always needed to restore proper lid position. Our Oculoplastic Surgeon works collaboratively with the dermatology or plastic surgery team involved in the original cancer care to plan reconstruction that addresses both the functional and cosmetic needs of the eyelid.
Ectropion can develop after a cosmetic lower eyelid procedure if too much skin was removed, leaving insufficient tissue to hold the lid in its correct position. Correcting this type of ectropion may require a skin graft or a midface tissue repositioning to replace what was lost. These revision cases tend to be more surgically complex than primary repairs, and an experienced, fellowship-trained Oculoplastic Surgeon significantly improves the likelihood of a good outcome.
These answers address practical questions patients often have after their consultation or while considering whether to move forward with repair.
For most patients, yes, though it can take a few weeks after surgery for drainage to normalize as the tissue settles and the tear system readjusts. If watering persists beyond three months despite a successful lid repair, the cause may be a separate blocked tear duct rather than the ectropion itself. A blocked duct is a distinct condition with its own straightforward treatment, and your Oculoplastic Surgeon can evaluate this at your follow-up if needed.
Drops, ointments, and lid-taping techniques can provide temporary relief but they do not correct the underlying problem. If you are applying lubricating drops many times each day, waking with significant eye pain, or noticing that your vision is being affected, waiting longer increases the risk of corneal damage. Your Oculoplastic Surgeon can assess how much the cornea has already been affected and advise whether prompt repair is recommended or whether a period of protective management is appropriate first.
The procedure itself is not painful because the area is fully numbed before any surgical work begins. Afterward, most patients describe a feeling of tightness or a dull ache around the outer corner of the eye rather than sharp pain, and this is typically managed well with acetaminophen. A sudden or significant increase in pain, swelling, or vision changes after you return home is not expected and should prompt a call to our office the same day.
Ectropion turns the eyelid outward so the inner surface is exposed and the eye dries out. Entropion turns the lid inward so the eyelashes rub directly against the cornea, which causes its own distinct pattern of irritation and damage. Both conditions require surgical repair, but the procedures, recovery, and risks differ between them. Some patients develop one condition in one eye and the other condition in the opposite eye, depending on how the supporting tendons have changed over time.
A carefully performed repair holds for many years in most patients. Recurrence is more likely in individuals with very fragile or thin tissue, ongoing facial nerve weakness, or untreated scarring that continues to tighten over time. Your Oculoplastic Surgeon can often anticipate higher-risk situations during the planning stage and incorporate additional support into the original repair to reduce the chance of the lid turning outward again.
You should avoid mascara, eyeliner, and anything applied directly along the lash line for approximately two weeks while the incisions are healing. Products applied to areas away from the surgical site are typically fine after about a week, but your surgeon will confirm this at your follow-up visit. Once you receive clearance, replacing your mascara with a fresh tube is a good idea to avoid introducing bacteria to a recently operated lid.

Dr. Mahboobi is amazing and even figured out my difficult multifocal contact lens issue so I can now see without readers. She even rearranged her schedule to make apmt fit along with my husband’s appointment for his post-op cataract apmt in which Dr. Reisner miraculously used laser to fix his cataracts and fixed his lifelong vision so he no longer needs contacts or readers. Life has improved tremendously for him thanks to Dr. Reisner’s laser cataract with trifocal IOLs.
