Your eyes depend on a network of tiny blood vessels to deliver oxygen to the tissues that help you see. When blood sugar stays elevated over time, those small vessels become weak, leaky, or may close off entirely. That is the starting point for most diabetic eye disease. The retina is the light-sensitive layer at the back of your eye. The macula is its central part, responsible for the sharp detail you use when reading, driving, and recognizing faces. Because both rely on a steady blood supply, they are often the first structures to show damage.

Short-term blood sugar swings are a normal part of living with diabetes; what shapes eye health most is the long-term average. Many people with diabetes also manage high blood pressure and elevated cholesterol, and both add strain to the delicate vessels in the retina. Our team often asks about your most recent readings and lab results. Both type 1 and type 2 diabetes can lead to eye disease. With type 2, changes can already be present at diagnosis, which is why a dilated exam is recommended shortly after you learn you have diabetes.

The Diabetic Eye Exam

A dilated eye exam is the cornerstone of diabetic eye care. Eye drops widen the pupils so your eye doctor can see the retina clearly. The exam is painless and usually takes less than an hour, though your vision may remain blurry and light-sensitive for a few hours afterward. For your first visit, it is a good idea to wear sunglasses and arrange a ride home. Most adults with diabetes should have this exam at least once a year, and more often if you already have signs of diabetic eye disease, if your blood sugar has been difficult to manage, or if you are pregnant and already living with diabetes.

Our team uses advanced imaging to capture detailed views of the back of your eye. Optical coherence tomography, or OCT, produces a cross-section image of the retina that reveals swelling or structural changes too subtle to see with the naked eye. Wide-field retinal photography captures a broad view in a single image, and comparing these images over time helps detect small but meaningful changes between visits.

Between visits, remember that diabetes does not always affect both eyes at the same rate. A simple way to monitor at home is to cover one eye at a time and look at a straight line, such as a door frame or window edge. If that line looks wavy, broken, or missing in either eye, contact your eye doctor promptly rather than waiting for your next routine visit.

Symptoms That Need Immediate Attention

Blurry or fluctuating vision, colors that look washed out, difficulty reading small print or seeing road signs clearly, trouble seeing in dim light, and a need for more light than usual during close work can all be early signs of diabetic eye disease. Mention any of these at your next scheduled visit.

Some changes should prompt you to contact your eye doctor right away, without waiting for your next appointment. Sudden vision changes can signal bleeding or swelling inside the eye, and eye pain with redness or nausea can signal a sharp rise in eye pressure. In either case, prompt care can make a real difference in the outcome.

Eye Diseases Diabetes Can Cause

Diabetic retinopathy

Diabetic retinopathy is the most common eye complication of diabetes. It occurs when blood vessels in the retina become damaged and begin to leak fluid or blood. In early stages there are usually no noticeable symptoms. In later stages, new and fragile blood vessels can form on the retina, leading to bleeding, scarring, and serious vision loss.

Diabetic macular edema

Diabetic macular edema (DME) is swelling in the macula caused by fluid leaking from damaged vessels, and it often develops alongside diabetic retinopathy. Because the macula handles your sharpest central vision, even mild swelling can make reading and close-up tasks difficult. DME is one of the leading causes of vision loss in adults with diabetes.

Cataracts

A cataract is a clouding of the natural lens inside your eye, the structure that helps focus light onto the retina. Everyone is at risk for cataracts with age, but people with diabetes tend to develop them earlier and sometimes more rapidly. Common symptoms include blurry or dim vision, increased glare from lights, and colors that appear faded.

Glaucoma

Glaucoma refers to a group of conditions that damage the optic nerve, which carries visual information from the eye to the brain. Diabetes raises the risk of certain forms of glaucoma, including a type linked to abnormal blood vessel growth inside the eye. Glaucoma usually has no early symptoms, which makes regular exams the most reliable way to detect it.

Not every vision change linked to diabetes signals lasting damage. When blood sugar shifts quickly, the lens inside the eye can temporarily swell and change shape, causing short-term blurriness that usually resolves once levels stabilize. If you are starting a new diabetes medication or your numbers have been fluctuating, let your eye doctor know before getting a new glasses prescription.

How Diabetic Eye Care Differs From Routine Eye Care

Routine eye care for someone without diabetes typically focuses on vision correction and general eye health. A diabetic eye care plan adds a structured schedule of dilated exams, retinal imaging, and careful documentation of any changes over time. Even when your vision is stable, these exams stay on your calendar because disease can progress silently.

At every diabetic eye visit, the retina and macula are examined carefully for tiny leaks, fluid buildup, new vessel growth, or early scarring. Retinal images are taken and stored so that changes can be compared side by side across multiple visits. Small findings that would not raise concern in a routine exam may be significant when diabetes is part of the picture.

Can diabetic eye disease be reversed?

Some early changes in the retina can improve when blood sugar and blood pressure are brought under better control. More advanced damage, such as significant scarring or established vessel growth, is generally not reversible, but treatment can still protect the vision you have and reduce the risk of further loss. The earlier a problem is identified, the more options you and your eye doctor have.

What is the difference between diabetic retinopathy and diabetic macular edema?

Diabetic retinopathy is the broader condition involving damage to the blood vessels throughout the retina. Diabetic macular edema is a specific complication where fluid leaks into the macula, the central part of the retina responsible for your sharpest vision. The two often occur together but can also develop independently, and each has its own treatment approach.

My vision feels fine. Do I still need a dilated exam every year?

Yes, and this is one of the most important points about diabetic eye disease. Because the retina has no pain sensors, significant damage can develop and progress without any noticeable change in how you see. By the time symptoms appear, the disease may already be at a stage where treatment is more complex. Annual exams are the most reliable way to find problems while they are still highly treatable.

How do I know if a vision change is an emergency?

Certain symptoms should prompt a same-day call rather than waiting for a scheduled visit: sudden loss of vision, a large increase in floaters, flashes of light, a shadow or curtain moving across your visual field, or eye pain combined with redness or nausea. These can indicate bleeding, a retinal detachment, or a sudden rise in pressure inside the eye, all of which require urgent evaluation.

Should I delay getting new glasses if my blood sugar has been unstable?

Yes, it is worth waiting until your blood sugar has been stable for at least several weeks. Fluctuating blood sugar causes the lens inside your eye to temporarily change shape, which shifts your prescription. A prescription measured during an unstable period may not reflect your true needs and could result in glasses that feel off once your levels settle.

How can I make sure my eye doctor and diabetes doctor are communicating?

You can take an active role in connecting your care team. Bring a brief written summary of recent exams and lab results to each appointment, and ask each provider how they share records with outside doctors. After your eye exam, you can specifically request that a summary be sent to your diabetes doctor, then confirm it was received.
