DCR can be performed through two main routes, and both are highly effective. Our Oculoplastic Surgeon will help you understand which approach suits your anatomy, your sinus health, and your personal preferences.
External DCR uses a small incision on the side of the nose, roughly half an inch long, positioned between the inner corner of the eye and the bridge of the nose. Working through this opening, the surgeon has a direct view of the tear sac and the surrounding bone. This approach has the longest track record and consistently high success rates in most reported case series.
Endonasal DCR uses a small camera and specialized instruments placed through the natural opening of the nostril. Because there is no skin incision, there is no external scar and typically less bruising. This approach is particularly well suited for patients with active sinus disease, since the nose can be evaluated and treated during the same procedure. Success rates are comparable to the external approach when performed by an experienced surgeon.
Both methods achieve excellent results in skilled hands. The decision depends on factors such as your nasal anatomy, any prior surgeries in the area, sinus health, and whether avoiding a small skin scar is a priority for you. A transcanalicular laser-assisted technique is also available in some practices, though reported success rates tend to be lower than either of the two primary approaches, so it is used less frequently.
Good preparation makes surgery go smoothly and helps recovery start off on the right foot. Your Oculoplastic Surgeon and our care team will walk you through every step well before your procedure date.
At your pre-op visit, your surgeon will review your current medications, allergies, and surgical history. If you take blood thinners, your prescribing doctor will need to advise whether pausing them is safe before surgery. A brief nasal exam is often performed at this visit to plan the best surgical approach for your anatomy.
DCR can be performed under general anesthesia or under local anesthesia with intravenous sedation. General anesthesia is commonly used for endonasal DCR because the camera work inside the nose requires the patient to remain completely still. Local anesthesia with sedation is a reasonable option for many external DCR procedures. Your surgeon and the anesthesia team will choose the approach that is safest for your individual health.
You will be asked to avoid eating and drinking after midnight before your procedure. Plan to arrive at least a couple of hours before your scheduled start time, and arrange for a trusted friend or family member to drive you home. Wearing comfortable, loose-fitting clothing that does not need to be pulled over your head makes getting ready much easier. Most patients are home within a few hours of arriving.
DCR is a precise procedure performed in a surgical setting. Understanding the main steps can help ease any anxiety before your appointment.
The central step of DCR involves removing a small piece of bone between the tear sac and the inside of the nose. This opening, roughly the size of a pencil eraser, becomes the new pathway through which tears will drain. Whether the approach is external or endonasal, this bone window is created in the same general location.
Once the bone window is made, the surgeon opens the tear sac and the adjacent nasal lining and stitches the two layers together so they heal as one continuous passage. Precise stitch placement is the most time-intensive part of the procedure and is critical to long-term success.
A soft silicone tube is placed through the newly created opening at the end of surgery in most cases. The tube keeps the new passage open while the surrounding tissues heal. It rests as a small loop at the inner corner of the eye and extends into the nose, and it is removed in the office, typically between three and six months after surgery. Removal takes only seconds and is not painful for most patients.
Most DCR procedures take between 45 and 90 minutes, depending on the surgical approach and your individual anatomy. When both tear ducts are blocked and both sides need to be treated at the same time, the operating time increases, but this combined approach is entirely reasonable when the situation calls for it.
Recovery from DCR is gradual, with most patients noticing meaningful improvement within the first few weeks. Knowing what to expect at each stage helps you heal comfortably and confidently.
Some bruising around the eye, mild swelling, and nasal stuffiness on the side of the procedure are normal in the first 48 to 72 hours. Sleeping with your head elevated on two pillows reduces swelling, and applying a cool pack for short intervals during the first day helps with comfort. Light pink drainage from the nose for two to three days after surgery is expected.
Bruising fades and most patients feel comfortable returning to desk work within about a week. It is important to avoid heavy lifting, bending forward at the waist, and straining during this period. You should not blow your nose for at least the first week. If you need to sneeze, keep your mouth open to reduce pressure in the nasal passage.
Tearing usually improves noticeably in the first few weeks, though a small amount of watering can persist while the new opening finishes maturing. Your surgeon will check healing at follow-up visits and may gently flush the passage to confirm it is open. During this period, the silicone tube will be removed in the office, a quick and simple step.
By six months after surgery, the new drainage pathway is stable for the vast majority of patients. Long-term success typically means comfortable, dry eyes with no recurring infections. A small number of patients experience narrowing of the opening due to scarring over time and may benefit from a minor revision procedure to restore full drainage.
DCR is a well-established procedure with a strong safety record, especially in experienced hands. As with any surgery, there are risks worth understanding before you proceed.
Bruising, mild bleeding from the nose, nasal congestion, and awareness of the silicone tube at the inner corner of the eye are all common in the early weeks after surgery. These are temporary and resolve as healing progresses.
A smaller number of patients may experience complications that require additional attention. These can include partial or complete closure of the new opening due to scarring or adhesions, a skin infection at the incision site in external DCR, displacement of the silicone tube before it is scheduled for removal, or a small visible scar after external DCR that varies in appearance from person to person.
Significant complications such as injury to nearby orbital structures, vision changes, or a leak of cerebrospinal fluid are rare and are associated with inexperienced technique. Our Oculoplastic Surgeon will review all relevant risks with you in detail during your consultation so you can make a fully informed decision.
These answers address practical questions that often come up after learning the basics of DCR, including what to expect in specific situations.
The tube rests as a small loop at the inner corner of the eye and is quite thin. Most people who do not know it is there will not notice it from a conversational distance. The tube is rarely painful, though some patients feel mild awareness of it in the early weeks. It is a temporary part of the healing process and is removed before the six-month mark in most cases.
Glasses can be worn immediately after surgery, but the nose piece may press on a tender area if you had external DCR. Using a soft cushion on the bridge or switching to a frame style with a different fit can help keep weight off the incision while it heals. Most patients find a comfortable solution within a few days.
DCR is specifically designed to open a blocked drainage pathway and will not improve tearing caused by dry eye disease. In dry eye, the surface of the eye is irritated and produces reflex tearing, which is a separate condition from a duct blockage. Some patients have both problems at the same time, and your Oculoplastic Surgeon will work with our team to identify and address all contributing causes before recommending surgery.
Most patients can resume CPAP use after a brief pause following surgery. The timing and any special guidance will depend on which surgical approach was used and how your healing is progressing. Using a saline nasal rinse as directed can help keep the inside of the nose moist and may make CPAP more comfortable in the first few weeks after the procedure.
Closure or narrowing of the new drainage pathway is the most common reason DCR does not provide lasting relief. If it occurs, a revision procedure may be considered to widen or recreate the opening. Regular follow-up visits during the first year after surgery help detect any narrowing early, when it is easier to address. Your surgeon will discuss the signs to watch for so you know when to reach out between scheduled appointments.
Contact your surgeon right away if you experience heavy bleeding from the nose that does not slow with gentle pressure, a sudden increase in pain or swelling several days after surgery, vision changes of any kind, or signs of infection such as increasing redness, warmth, or discharge around the incision. These situations are uncommon but should be evaluated promptly rather than managed at home.

I've been seeing Dr. Reisner for over 20 years for all of my eye care needs. He did my Lasik surgery 20 years ago, treated my extremely dry eye issues, took care of my eyeglass prescriptions, did both of my cataract surgeries, and today surgically removed a calcification from my lower eyelid. He is an excellent doctor in every way (kind, patient, great listener, etc.). Staff are always kind and professional I absolutely recommend Dr. Reisner and The Eye Center. Update: Another visit (Fairfax office) and saw Dr. Tailor for a stye problem She is friendly, patient and answered all of my questions. I did not feel rushed at all. Staff wonderful. Very good practice highly recommend.
