Cataract surgery often has a positive effect on glaucoma beyond simply clearing your vision. Understanding what changes after surgery helps set realistic expectations and supports better long-term planning.
Many patients experience a modest drop in eye pressure following cataract surgery. This happens because removing the natural lens creates more space for fluid to drain from the eye. The amount of pressure reduction varies from person to person, and it is not guaranteed, but for many glaucoma patients this change is a meaningful benefit.
For patients with mild to moderate glaucoma, a combined approach is often recommended. The cataract surgeon performs cataract surgery and a minimally invasive glaucoma surgery (MIGS) procedure in the same session. MIGS procedures work by improving the eye's natural drainage system to lower pressure with a smaller risk profile than traditional glaucoma surgeries. Combining the two procedures means one recovery period instead of two.
Not every glaucoma patient needs a combined procedure. If your glaucoma is mild and well controlled on drops, cataract surgery alone may be the right approach. Your cataract surgeon will review your pressure history, optic nerve status, and overall eye health before recommending a plan.
Patients with advanced glaucoma may need a more involved glaucoma procedure, such as a trabeculectomy (a surgical opening created in the eye wall to drain fluid) or a tube shunt (a small device implanted to direct fluid out of the eye), rather than a smaller MIGS approach. These procedures may be performed at the same time as cataract surgery or in separate sessions, depending on what is safest for your eye.
Patients with glaucoma require more detailed pre-surgical evaluation than those without the condition. Several additional steps help your cataract surgeon build a plan that is safe, precise, and suited to your eye's specific needs.
Your care team will carefully review your eye pressure records over time. Patterns of pressure fluctuation, any history of pressure spikes, and any prior glaucoma procedures all factor into the surgical approach. A long and stable pressure history is a reassuring sign going into surgery.
Imaging of the optic nerve and a visual field test (a computerized exam that maps your peripheral vision) help determine how much glaucoma damage has already occurred. This information guides both the choice of lens and the surgical strategy. More advanced damage may point toward a simpler, more conservative surgical plan.
Most patients with glaucoma are best served by a standard monofocal lens, which provides clear focus at one distance. Multifocal lenses divide incoming light to allow for multiple focal points, but this can reduce contrast sensitivity, which is especially problematic for eyes with optic nerve damage. Toric lenses, which correct astigmatism while providing clear distance vision, are often a good option for glaucoma patients who need astigmatism correction. Your cataract surgeon will discuss which lens type is appropriate for your eyes and your goals.
Some glaucoma drops are continued right through the surgical period, while others may be paused briefly. Your care team will give you a clear written schedule so you know exactly which drops to take, when to take them, and when to resume any that were temporarily stopped. Following this plan closely helps keep your pressure stable before and after surgery.
Cataract surgery in a glaucoma patient follows the same basic steps as standard cataract surgery. When a MIGS procedure is added, the total time in the operating room increases only slightly, and recovery is largely the same.
Numbing drops are placed in the eye before surgery begins. The cataract surgeon makes a small incision at the edge of the cornea, then uses a technique called phacoemulsification (sound waves that break up the cloudy lens) to remove it. The new artificial lens is inserted through the same small opening. If a MIGS procedure is part of the plan, it is performed through the same incision during the same session. The surgery is performed in our accredited in-office surgery suite, so there is no need to travel to a hospital or separate surgical center.
Most patients remain awake during cataract surgery, and light sedation is available to help with relaxation. You may notice bright lights or a sense of pressure during the procedure, but significant pain is uncommon. The team communicates with you throughout to keep you comfortable and informed.
Vision is blurry immediately after surgery and gradually clears over the first several days. You will use antibiotic and anti-inflammatory eye drops as directed, and your glaucoma drops will continue on the schedule your care team has outlined. Mild scratchiness, light sensitivity, and watery eyes are normal in the early days of healing. Eye pressure is monitored more closely in glaucoma patients during recovery, with follow-up visits scheduled to track how your pressure responds. Most pressure fluctuations settle within four to six weeks, and a new glasses prescription is typically ready around that same time.
Successful cataract surgery does not end the need for ongoing glaucoma monitoring. Glaucoma is a lifelong condition, and regular eye exams and pressure checks remain essential even after surgery. Drop regimens may need to be adjusted over time as your pressure patterns and optic nerve status change.
The large majority of cataract surgeries go smoothly, but patients with glaucoma carry a slightly elevated risk profile compared to the general population. Knowing what to watch for helps you get prompt care if something does not feel right.
Mild scratchiness, light sensitivity, and watery eyes are expected in the first few days and typically resolve on their own. Some patients notice halos or glare around lights at night during early healing. These effects usually fade as the eye heals over the first few weeks.
Eye pressure can rise in the early weeks after cataract surgery, and this is more likely in patients who already have glaucoma. Your care team will check pressure at each follow-up visit. Most pressure spikes are treated with a temporary adjustment to your drop regimen. Occasionally, a brief in-office procedure is needed to bring pressure back down.

These symptoms can indicate a pressure spike, infection, or retinal problem, and prompt evaluation leads to better outcomes.
The most common long-term complication after cataract surgery is posterior capsule opacification (PCO), which is a cloudiness that can develop on the thin membrane behind the new lens months or years after surgery. PCO is treated quickly and painlessly with a laser procedure called a YAG capsulotomy, performed in the office. Because this laser can cause a brief rise in eye pressure, glaucoma patients should have their pressure checked at the follow-up visit after a YAG procedure.
These answers address the questions our patients most often ask when they are navigating both cataracts and glaucoma.
No, cataract surgery does not cure glaucoma. It often lowers eye pressure meaningfully, and that reduction can reduce how many drops you need, but it does not halt the underlying disease. Glaucoma requires lifelong monitoring and management regardless of how well surgery goes.
For patients with mild to moderate glaucoma, combining cataract removal with a MIGS procedure is well supported. It avoids the need for a second surgical recovery and has a favorable safety profile. Whether a combined approach is right for you depends on the severity of your glaucoma, your pressure history, and your overall eye health, all of which your cataract surgeon will evaluate carefully.
Multifocal lenses are generally not recommended for patients with moderate or advanced glaucoma because they reduce contrast sensitivity, which becomes more noticeable when the optic nerve has already suffered damage. Patients who want to correct astigmatism alongside their cataract may be good candidates for a toric monofocal lens, which provides excellent distance clarity without the contrast trade-off. The best lens choice for your eye will be discussed in detail during your consultation.
This depends on how your pressure responds after surgery. Some patients achieve enough of a pressure reduction to reduce their drop burden, and a smaller number are able to stop drops entirely. Others continue with a similar regimen as before. Your cataract surgeon will monitor your pressure carefully in the weeks after surgery and adjust your medications based on actual results rather than expectations.
If your glaucoma is severe or difficult to control, a MIGS procedure may not provide enough pressure reduction. In that case, your cataract surgeon may recommend a more comprehensive glaucoma surgery, such as a trabeculectomy or tube shunt, either at the same time as cataract surgery or as a separate procedure. The decision depends on how much pressure control your eye needs and how urgent the glaucoma situation is relative to the cataract.
Most insurance plans, including Medicare, cover cataract surgery when it is medically necessary. Coverage for MIGS procedures varies by plan, device type, and whether glaucoma is documented in your records.
