As we get older, lacrimal gland function naturally declines, producing less tear fluid over time. Hormonal changes, particularly during menopause, can accelerate this by disrupting the biological signals that stimulate tear production. This is a major reason aqueous deficient dry eye becomes more common in adults over 65 and in postmenopausal women.
Several commonly prescribed and over-the-counter medications can reduce tear production as a side effect. These include antihistamines, certain blood pressure medications, antidepressants, and some acne treatments. If you notice dry eye symptoms after starting a new medication, it is worth mentioning this to both your prescribing doctor and your eye doctor, as alternatives may be available.
Sjogren's syndrome is one of the most well-known causes of aqueous deficient dry eye, as it directly attacks the moisture-producing glands throughout the body. Rheumatoid arthritis, lupus, thyroid disorders, and diabetes can also impair tear gland function, sometimes in ways that appear in the eyes before other symptoms become obvious. Managing these conditions in partnership with your broader medical care team often leads to improvement in eye comfort as well.
Low indoor humidity, air conditioning, wind, and smoke can all accelerate tear loss and make underlying aqueous deficiency more apparent. Prolonged screen time reduces how often you blink, which is how tears are spread evenly across the eye surface. Contact lens wear and certain eye surgeries can also temporarily reduce tear production and may require additional support during recovery.
The symptoms of aqueous deficient dry eye can range from mild nuisance to noticeable daily interference. Catching these signs early and connecting them to their cause helps prevent the condition from progressing and makes treatment more straightforward.
The most common description is a gritty or sandy feeling in the eyes, as if something is lodged beneath the eyelid. Burning or stinging sensations are also frequent, especially in dry or air-conditioned environments. These feelings often build throughout the day and may ease temporarily with eye drops or rest.
When tears are insufficient to keep the surface of the eye smooth and evenly coated, vision can become intermittently blurry. This blur often clears after blinking but returns quickly, especially during reading or focused close-up work. Light sensitivity and increased glare, particularly noticeable during night driving, are also common complaints linked to an unstable tear film.
Eyes affected by aqueous deficiency often appear red and feel tired more quickly than usual, even with modest amounts of screen use or reading. Rubbing the eyes for relief can worsen the irritation and increase the risk of further surface inflammation. Many people also find they squint more often or feel a need to avoid brightly lit spaces.
Contact lenses rely on a healthy tear film to remain comfortable throughout the day. When aqueous production is low, lenses may feel dry, scratchy, or irritating within hours of being placed, sometimes making regular wear difficult to sustain. This is a common signal that prompts many patients to seek an evaluation of their tear production.
Our team uses a combination of a thorough history, clinical examination, and targeted testing to confirm aqueous deficient dry eye and determine how severe it is. These evaluations are comfortable, non-invasive, and give us the information we need to create the right plan for your eyes.
We begin by asking detailed questions about your symptoms, how long they have been present, your medical history, and the medications you take. A slit-lamp examination, which uses magnification and a focused beam of light, allows us to look closely at the eyelids, eye surface, and the quality of your tear film. This overview often points directly to the type of dry eye involved before testing begins.
The Schirmer test is a straightforward way to measure how much tear fluid your eyes produce. A small strip of paper is gently placed under the lower eyelid, and after a few minutes we measure how much fluid soaked into it. We also assess tear breakup time, which tells us how quickly the tear film begins to break apart after a blink, giving us a clear picture of tear stability.
Dyes such as fluorescein and lissamine green can be applied painlessly to the surface of the eye and reveal areas where dryness has caused microscopic damage to the cornea or conjunctiva (the clear tissue covering the white of the eye). These staining patterns help us understand how far the condition has progressed and how urgently treatment is needed.
For more complex cases, imaging tools such as meibography allow us to visualize the oil glands in the eyelids and confirm whether evaporative factors are also at play alongside the aqueous deficiency. Testing for inflammatory markers on the ocular surface can identify cases where targeted anti-inflammatory treatment is needed. These tools help us build a more complete and precise picture of your condition.
Treatment is tailored to the severity of your condition and its underlying cause, ranging from simple daily habits and over-the-counter products to prescription medications and in-office procedures. Consistent use of the right combination of therapies gives most patients meaningful and lasting relief.
Preservative-free artificial tears are the foundation of dry eye management for most patients. They supplement the natural tear film and provide quick, temporary relief from discomfort. Thicker gels and ointments are particularly helpful at night, when tear production naturally slows and the eye surface can become quite dry by morning.
Prescription drops such as cyclosporine (Restasis) and lifitegrast (Xiidra) work by calming inflammation within the lacrimal glands, which allows them to produce more natural tears over time. A newer option, Tyrvaya nasal spray, stimulates tear production through a different pathway involving nasal nerves. These medications typically take several weeks of consistent use before their full benefit is felt.
Punctal plugs are tiny devices placed into the small drainage openings in the inner corners of the eyelids, slowing the drainage of tears and keeping more moisture on the eye surface. The placement is performed comfortably in our office using numbing drops and takes only a few minutes. Temporary collagen plugs dissolve on their own over time, while longer-lasting silicone options can be removed if needed.
Small adjustments at home and work can meaningfully reduce symptoms. Using a humidifier, taking regular screen breaks, applying warm compresses to the eyelids, and staying well hydrated all support better tear function. Omega-3 fatty acid supplements, found naturally in fatty fish, walnuts, and flaxseeds, may also help reduce the inflammation that contributes to gland dysfunction.
For patients who do not respond adequately to standard therapies, more specialized options are available. Autologous serum eye drops, made from your own blood, contain growth factors that support healing of the eye surface. Intense pulsed light therapy targets inflammation affecting the eyelid glands. Scleral contact lenses create a fluid reservoir over the eye surface, providing continuous moisture and protection in severe cases.
Patients often have questions about how aqueous deficient dry eye fits into their daily life, their overall health, and their long-term eye care. These answers are designed to help you think through the practical decisions involved in managing this condition.
If it goes untreated for an extended period, low tear production can lead to damage on the surface of the cornea (the clear front part of the eye), increasing the risk of scarring or infection. However, most patients who begin treatment before significant damage occurs do very well with consistent management. This is why we recommend not waiting too long before seeking an evaluation, even if symptoms feel mild.
Many patients with aqueous deficient dry eye can continue wearing contacts, but it often requires some adjustment. Lenses designed specifically for dry eye conditions, along with preservative-free lubricating drops used before and during lens wear, can improve comfort significantly. If discomfort persists despite these measures, your eye doctor may recommend reducing wearing time or transitioning to glasses while treatment takes effect.
Sjogren's syndrome is an autoimmune disease that attacks moisture-producing glands throughout the body, with the lacrimal glands being among the most commonly affected. Signs of Sjogren's-related dry eye sometimes appear during a routine eye exam before a patient is even aware they have the condition systemically. If our team identifies signs that suggest an autoimmune cause, we may recommend coordinating your care with a rheumatologist for a more comprehensive approach.
Most patients benefit from check-ins every six to twelve months to monitor tear production and make any needed adjustments to their treatment. Patients with autoimmune conditions or more severe symptoms may need to come in more frequently, especially during the first year of treatment when we are fine-tuning the approach. Regular follow-up also allows us to catch any surface changes early, before they become more difficult to treat.
Tear production does tend to decline gradually with age, so proactive management is more effective than waiting for symptoms to become severe. Establishing a consistent treatment routine early, including lubricating drops, lifestyle adjustments, and any recommended prescription therapy, helps slow the progression and maintain comfort. Annual or biannual evaluations allow us to adapt your care plan as your needs change over time.
Cold, dry winters and hot, windy summers can both trigger flare-ups by accelerating tear evaporation and adding environmental stress to already limited tear production. Using a humidifier indoors and wearing wraparound sunglasses outdoors can help buffer these seasonal effects. Patients often find they need to use lubricating drops more frequently during these times, and discussing a seasonal adjustment to your treatment plan with our team is always a reasonable option.
